Service Description:	Education, Self-Management and Exercise Program for adults with history of cardiac disease Offers a 3-month program for local patients; 6 month program for Northern patients Provides assessment, stress test, exercise prescription, exercise sessions, and discharge completed on site Option for in-home program (Northern patients) Telemedicine connection for education sessions Access to dietitian, social worker, and smoking cessation
