Service Description: |
Community-based support and rehabilitation services to adults living with the effects of an acquired brain injury
Brain Injury Outreach Program - Rehabilitation workers meet with people with acquired brain injuries in their home or community to provide community-based rehabilitation to promote independence in various areas of day to day functioning (physical, cognitive, behavioral, social, activities of daily living) * offers various educational, rehabilitation, support and leisure groups Community Trauma Program - Staffed by a social worker with access to an occupational therapist, available to people who have experienced a traumatic injury requiring assessment, private or family counselling, service coordination and rehabilitation services ABI Congregate Care - Offers a 2 bedroom shared living environment for people who require 24 hour care * provided while promoting on-going rehabilitation, support and supervision Post Transitional Stroke Program - Services include a Stroke Community Navigator who supports individuals post discharge and residing in the community to access support services based on personal needs * services can include cognitive and physical therapy provided by rehabilitation support workers and overseeing therapists |
