Service Description: Nutrition, transportation, in-home and wellness services to support the health and well-being of people in Northumberland County
 
Friendly Visiting Volunteer provided on a one to one basis to visit on a regular basis * visit activities may be playing cards, walks, crafts, conversation * volunteers are screened * can provide service for persons with dementia
 
Home At Last - Hospital Transition Program Transitional supports when being discharged from hospitals in Northumberland County
 
Home Help and Maintenance Program Assistance with light cleaning * shopping * meal preparation
 
Hospice Palliative Care Program - Visiting Hospice Service Trained volunteers provide emotional, psychological, spiritual and social supports
 
Meals On Wheels Nutritious hot or frozen meals are delivered during the noon hour by volunteers to the person's home * special diets can be accommodated
 
Personal Distress Alarms Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response
 
Specialized Transportation - Accessible and Rural Services Transportation to medical appointments, shopping and social activities and programs
 
Support and Care for Caregivers Assistance with the on-going responsibilities of caregivers who are caring for a loved one, friend or neighbor * peer support and discussion for caregivers and family and friends to share coping strategies and information on resources
 
Telephone Reassurance Calls Regular telephone contact provided in order to provide reassurance and arrange for assistance if necessary * normally short five minute calls at regular, prearranged times or when family and friends are away
 
Wellness Programs Social and recreational activities * program offerings vary
