Service Description:	Telephone Reassurance - pre-scheduled phone calls, daily service, for adults 55+ supporting independent living * quick emergency response when required * assistance in accessing community resources Benefits of scheduled check-in calls: supports health routine reminders (e.g. medication, meals, exercise, appointments) provides social connection increases safety, health and well-being quick link to emergency response when required and to community resources when required support to caregivers and those living with dementia ease concerns for family
