Service Description: Service for older adults and adults with health challenges like depression, anxiety, or other mental health concerns.

Service include:
  • an intake assessment-in client home or other convenient location
  • short or long term case management-building a supportive partnership, providing assistance in managing illnesses, and linking with other community resources


A partnership initiative between CANES Community Care, Canadian Mental Health Association, Peel Dufferin Branch and Reconnect Community Health Services

Application: Referrals can be made by an individual, a family member, or a professional; by telephone, fax or sent electronically by downloading the referral form from CANES website
Eligibility - Population(s) Served: Seniors living in the Etobicoke, Woodbridge, Malton and Brampton areas (Ontario Health atHome-Brampton (Central West) Seniors (65 yrs and up) with a mental health issue (dementia excluded). Adults over 55 yrs can also be seen if they have complex medical issues in addition to the mental health issue
