Service Description: Offers various workshops in the community that supports both patients and their caregivers in their journey of managing chronic health conditions like chronic pain, diabetes, heart disease, arthritis, cancer and lung disease * Participants and their caregivers develop new tools and skills to help them manage the emotional, social and physical aspect of living with a chronic condition, or supporting someone with a chronic health condition.

Workshops offered include:
  • Living a Healthy Life with Chronic Pain
  • Living a Healthy Life with Diabetes
  • Living a Healthy Life with Chronic Conditions
  • Better Choices, Better Health®, an online Chronic Disease Self-Management Program for participants across Ontario
  • Powerful Tools for Caregivers
  • Getting the Most from your Healthcare Appointment

Also offers workshops for health care providers to enhance their knowledge and skills in supporting self-management behavior for their patients.

Health care provider workshops include:
  • Choices and Changes: Motivating Healthy Behaviour 
  • Brief Action Planning
  • Mentorship sessions including motivational interviewing workshop series and webinars

Application: Call * no referral required for most services * referral through school for in-school services
