Service Description:	Offers a 6-week workshop for individuals living with chronic or long-term health conditions such as diabetes, arthritis, stroke, cancer, colitis, lung disease, fibromyalgia, Parkinson's, heart disease and other conditions Educates individuals on how to manage their symptoms to become more confident and improve their quality of life Offers workshops online Workshop topics include: Coping with difficult emotions Pain and fatigue management Developing action plans and problem solving skills Exercise and nutrition Managing Medications Making treatment decisions Working with the health care team

Application:	Registration is required Call the toll-free number or register on the website Note: Due to COVID-19, in-person workshops have been suspended. Register online to be added to a wait list and be notified when a workshop becomes available.
Eligibility - Population(s) Served:	Individuals 18 years of age and older with chronic or long-term health conditions and their caregivers Participants must be able to set goals and function in a group setting; for example, the program would not be appropriate for someone with significant dementia or memory impairment, or for someone who would be disruptive in a group
