Service Description:

Registered Nurses (RN) with extensive knowledge and experience assist patients and families as their Care Manager for both acute situations and chronic conditions

Holistic approach may include:

community support, nursing, palliative care
accompaniment to medical appointments
oversight of treatments and procedures
communication with physicians and other providers
assistance with access to community resources
research diagnoses and treatment options
investigation of alternatives based on patient's needs
specialty in Parkinson's Disease management, including EMBRACE PD Clinic
Palliative Care and support with End-of-Life care needs
