Service Description: Coordinates all admissions into long-term care homes (including short-stay and respite care beds) and into complex continuing care (chronic care) facilities across Champlain * refers placement applications and assists in the coordination of admissions of long-term care facilities in other regions of Ontario

Application: Individuals can phone directly or they can be referred by family, friends or another professional * Referrals for placement services are accepted from institutions, agencies, physicians, hospitals, individuals in the community * Professional staff visit, counsel and assess referred persons and determine if they meet the eligibility criteria for placement/admission to a long-term care facility
