Service Description:	Adult Day Program for adults residing in their own homes opportunities to socialize, participate in activities and enjoy a warm meal snacks provided recreation, activation, socialization and enjoyment stimulates all aspects of an individual’s overall well-being (physical, intellectual, education, social, emotional) modified activities to meet individual needs caregiver/client support

Fees:	Set fees per day (includes hot meal and transportation)

Application:	Self-referrals accepted * evaluation required

Eligibility - Population(s) Served:	Adults living in the community who are physically frail, or who have a cognitive or physical impairment
