Eligibility criteria determined by Ontario Health atHome Waterloo-Wellington Eligibility includes: medically stable * able to tolerate 14 days of isolation * willingness to consent and complete COVID 19 surveillance, testing upon admission to the program and as required/ directed by the Wellington Dufferin Guelph Public Health * must be smoke-free and substance-free * up to 1 person may provide physical assistance for activities of daily living |
