Service Description:	Supports for Adults living with prediabetes or diabetes Education Clinics provide: certified diabetes educators , registered dieticians and nurses who work in cooperation with physicians, pharmacists and family members individual and group appointments referrals and monthly telehealth sessions with specialists for advanced care Education on: diabetes self-management healthy living and eating hyperglycemia (high blood sugar) hypoglycemia (low blood sugar) common emotions physical activity medications
