Service Description: Relief for caregivers of older adults with Alzheimer's disease and related dementias * 12-bedroom short stay respite residence

A safe and supportive environment where guests can stay for a few hours or 24 hours a day, up to 90 days a year

The service is flexible and can provide preplanned daytime or overnight accommodations

Unplanned admissions for previous guests are also permitted provided there are rooms available

Application: Contact an Ontario Health atHome Care Coordinator at 613-745-5525 and ask for the Respite House * If an applicant does not have an Ontario Health atHome Care Coordinator, ask to speak with the intake person to arrange for an assessment to determine eligibility

Once eligibility is determined, a referral is made to the Respite House Team Leader* An on-site visit to the Respite House for both the person with dementia and their caregiver will be arranged * The caregiver will assist in the development of an individualized plan of care for the prospective guest * Trial day and overnight stays must be completed before booking additional overnight stays
Eligibility - Population(s) Served: Applicants must be in the early to mid-stages of dementia, and medically stable * They must not be at risk of harming themselves or others, and not be bedridden or require full assistance with activities of daily living

Residency Requirements: Champlain territory which includes Renfrew County, the City of Ottawa, Prescott and Russell, Stormont, Dundas and Glengarry, North Grenville and four parts of North Lanark
