Service Description:	Transition support from school life to adulthood for those with developmental needs Program initiatives: support of a coordinator for the participant and their family as well as for the other agencies that are involved assessment of skills and abilities use of the Person Directed Planning model in creating and maintaining goals for the future customized supports and resources based on needs and interests
