Service Description: For patients living with COPD or chronic heart failure, Telehomecare can help

Through a telephone and the internet, patients will be linked with a registered nurse at Brampton Civic. The nurse will:

  • remotely monitor patient's health at home
  • contact patient to provide support and advice if patient's condition changes

This program is funded by the Ministry of Health and provided in partnership with the Ontario Telemedicine Network

Eligibility - Population(s) Served: Patients with an established diagnosis of a congestive heart failure (CHF) or chronic obstructive pulmonary disease (COPD)
Repeated emergency department visits and/or hospital admissions
Capable of using in-home monitoring equipment
Languages: English ; Hindi
Area(s) Served: Toronto (Etobicoke), Vaughan (Woodbridge), Dufferin County, and Peel Region
