A team of trained advanced practice providers assess patients and recommend treatment options * patients receive rapid low back pain assessment (less than four weeks on average), education and evidence-based self-management plans * referral into the program is only available to patients whose family doctor or primary care nurse practitioner has enrolled in the program Low Back Pain Rapid Access - Patient Intake Form Primary Care Providers must register and complete an online module to obtain referral privileges |
