Service Description: A specialized service of Ashby Community Support Services that focuses on individuals with co-occurring ABI and addictions and/or mental issues.

Services include:
  • comprehensive functional assessment in the home and the community
  • intensive case management and individualized community support services geared to the expressed needs and goals of the individual
  • 1:1 addiction counselling related to substance use if required
  • access to SUBI (substance use and brain injury) group
Essential community outreach services continue in person as needed. Virtual support is an option where appropriate
Fees: Set fee * subsidy available for eligible participants
