Multidisciplinary clinic * rapidly diagnoses breast conditions and supports patients * Osler health care team works with primary care physicians to manage and coordinate care The DAP consists of:
Patients in this program have access to a team of Osler radiologists, pathologists, surgeons, and medical and radiation oncologists. A dedicated nurse navigator arranges rapid consultation and treatment if a biopsy is indicated. The nurse navigator also provides education and guides patients through care transitions, while keeping the primary care physician informed of the patient’s progress Physician requisition forms and referral information for Diagnostic Imaging can be found in the Find a Doctor of Service directory |
