Service Description: Volunteer Visiting program that supports adults with a life-limiting illness

Services are delivered in the client's home, retirement home, long-term facility or hospital

Services provided by a team of trained volunteers along with two Registerd Nurses  * the Coordinator of Visiting Volunteers, a Registered Nurse, makes an initial holistic assessment visit before matching the client with a Visiting Volunteer

The Home Visiting Team can also provide caregiver support, bereavement support, information, referral to other community services and advocacy and helps with navigating the healthcare system

All services are confidential and free of charge

Member of the Hospice Palliative Care Ontario (HPCO), North Lanark Palliative Care Network and the Champlain Hospice Palliative Care Program

The Coordinator refers clients to the Ontario Health atHome Home Care Program for palliative care coordination and nursing care, Champlain Palliative Consultation Team for pain and symptom management and to a physician for medical needs

Collaboration agreement with the Paramedicine Program of Lanark Leeds Grenville for referrals to do in-home health checks as required
