Service Description: Services include:
  • personal support services by Personal Support Workers (PSW), Developmental Service Worker (DSW), Community Service Worker (CSW) and Home Support Worker (HSW)
  • nursing services - Registered Nurse (RN) or Registered Practical Nurse (RPN)
  • allied services - Occupational Therapist (OT), Physiotherapist (PT), Speech Therapist (SLP), dieticians, social worker and recreational therapist
  • transitional and ambulatory care service

Services can be provided in the home, hospital and various other settings
Fees: Rate start from $28 per hour
Application: Intake Assessment Form to be completed
Eligibility - Population(s) Served: Adults, individuals with disabilities and seniors requiring assistance
