Service Description:	Provides short term client centered case management from an integrated model of care. This includes, information and education around harm reduction (which includes abstinence), substance use, distribution of harm reduction equipment, advocacy, care coordination, referrals to appropriate programs/services and groups, such as relapse prevention.

Application:	Referrals are accepted from anyone, including self-referrals. Please call to make a referral.
