Service Description: Free six-week workshops for people with chronic health conditions and caregivers including:

Living a Healthy Life with Chronic Conditions - for anyone with a chronic condition * includes diabetes, heart disease, arthritis, lung disease, and other chronic health issues

Living a Healthy Life with Chronic Pain - for anyone suffering from pain, which could include but is not limited to: low back pain, pain for motor vehicle accidents, arthritis, fibromyalgia, etc * The Moving Easy Program is a gentle movement program and offered as part of the workshop

Living a Healthy Life with Diabetes - for anyone with diabetes, in particular people with Type 2 and pre-diabetes

Powerful Tools for Caregivers - designed to help family caregivers take better care of themselves while caring for a family member or friend

Interactive topics include physical activity, healthy eating, medication management, future planning, communicating with your health care provider, action planning, depression management, and pain management

Application: Register online @ www.healthy-living-now.ca/upcoming-workshops * anyone can register themselves, no referral from a health care professional is needed
Eligibility - Population(s) Served: Open to Ontario Health atHome Central area residents with chronic conditions, caregivers, friends and family of people with chronic conditions, as well as the general public
