Service Description: Provide support to individuals who are living with the effects of a mild/moderate - severe acquired brain injury in working on their personal goals * work with our client to develop tailored supports and strategies to facilitate their success in reaching their goal(s)

Following the assessment process, recommendations, and a plan for specific individualized service needs and goals are developed * depending on the goals that have been established, the length of the program could be 6-12 weeks long * further assessments may be conducted on an as-needed basis

Service is provided in home, community, school or workplace - once or twice a week (depending upon the need)

Coaches assist clients to set their individual goals * goal areas could include:
  • organization and planning
  • budgeting or financial management
  • education or employment support
  • community orientation and travel training
  • self care/daily routines/stress management focusing on health and overall well-being
  • building social and leisure recreational options
Work to assist individuals in the development of their personal support network and to be able to access the community and for them to broaden their community involvement

Eligible Candidates:
  • 18 years of age or older
  • mild/moderate acquired brain injury (must provide medical documentation confirming diagnosis);
  • able to work on short-terms goals and make a commitment to the program
  • do not present a harm to themselves or others
  • able to interact safely when out in the community
