Service Description: Adult day program * social, recreational, and therapeutic activities for frail elderly adults and for people with Alzheimer's disease and other related dementia in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

Programs are designed to meet a variety of needs and interests and assist people to achieve and maintain their maximum level of functioning, postpone or prevent institutionalization

  • daily exercise program

Eligibility - Population(s) Served: Older adults, adults with disabilities, and people with Alzheimer's Disease or related dementia * must be able to tolerate a group setting, be medically stable, not pose a risk to self or others, be able to safely perform a one-person assisted transfer if assistance is required * must have a valid health card * care coordinators assess the needs of applicants
