Self-management, education, exercise, and medication services for adults dealing with chronic pain * interdisciplinary team helps patients with adaptive changes to improve daily functioning, productivity and quality of life

Maintenance level of rehabilitative care for patients working to prevent functional decline/injury or maintain functional performance * programs include:
