Service Description:	The regional renal program provides critical kidney care to patients across the Central West region, offering a wide range of kidney care services, including: early diagnosis, treatment and monitoring treatment counselling dialysis preparation for kidney transplants and dual kidney/pancreas transplants Depending on patient needs: connect patients with the services and supports they need, both at Osler and at partner facilities support and follow patient progress every step of the way offer services such as home dialysis, when possible, to promote a flexible, independent lifestyle Osler's renal program partners with: St. Michael's Hospital and University Health Network (for transplantation services) Ontario Health atHome, Brampton - Central West Region The Kidney Foundation of Canada
