Service Description:

Program launched by Dufferin County Paramedic Service, in partnership with Ontario Health atHome-Brampton (Central West) and Headwaters Health Care Centre, helps seniors, the disabled and people with chronic illnesses get the care they need * Non-emergency, community-based service with a focus on health promotion, system navigation and injury prevention

Serves high risk patients living with complex medical issues such as diabetes, COPD, CHF and co-morbidities

May provide the following services:

wellness checks
safety home assessments
ability to refer to community partners
in home blood work
urine tests
in consultation with primary care physicians, administration of medications for various conditions
remote Patient Monitoring (RPM)
