Service Description:	Inpatient consult service is available to patients admitted to hospital and their families Offer: recommendations to optimize pain and other symptoms information around what to expect, discussions regarding options, and establishing goals of care transfer to the acute palliative care unit for management of complex symptoms or end of life care (when indicated) support with planning for palliative care at home or other facilities follow up in outpatient clinic Location: 5th floor, North Building, Entrance A, take the ROSE elevators
