Service Description:	Support for those caring for a loved one at home or from afar: develop and evolve your personalized care plan with you participate in individual counselling and support groups assist you with referrals and in connecting with other community caregiver services support you in the transition to grief and bereavement supports participate in wellness activities (e.g. yoga, reiki, etc) participate in educational workshops (based on community need)
