Service Description: |
Meets the needs of older adults with cognitive disorders Seniors Mental Health Outreach - specialty team providing comprehensive assessment, treatment recommendations, care planning, advocacy, referrals, and education * team includes case managers, registered nurses or occupational therapists, and psychiatrists * offered through Providence Care for Kingston, Frontenac, Lennox and Addington, and through the The Royal Hospital for Lanark, Leeds and Grenville Mobile Response Teams - support for residents living in long-term care homes who are presenting with or at risk for responsive behaviours * assists with assessments, behavioural care planning and transitional support to meet specific resident needs * overnight telephone support for Long-Term Care is available Psychogeriatric Resource Consultants - provide education, consultation and community development for health care providers across the health care system Services and Programs: |
