Service Description: One-on-one in-home gently exercise program for clinically frail, home-bound older adults * aims to maintain and improve their fitness and independence, to increase the strength and balance, and to reduce the risk of falling * instructor or volunteer facilitates a standardized program of up to 15 gentle exercises that can be modified for all levels of ability * the goal of the program is for the participant to progress enough to be able to join the group exercise program upon completion of the in-home program

Program criteria:
  • client who have received recent physiotherapy specific to mobility declines will be excluded
  • client must have functional goals that can be addressed through standard exercise
  • client must be unable to attend community group exercise classes
  • client's goal must be to transition to group exercise or continue independently with exercises

Application: Complete and fax in the referral form and client health information sheet
Eligibility - Population(s) Served: Adults 65 and older * discharged from physiotherapy * have a barrier to a group exercise class * have an Assessment Urgency Algorithm (AUA) between 2-5 * must have functional goals that can be addressed through standardized exercise with hopes of transitioning to group exercise classes
