Cardiac Rehabilitation at Home (Progression) Virtual cardiac rehabilitation program designed for those who cannot attend an on-site program. This program combines exercise counselling, education and self-management tools to support lifestyle changes and goals
 
Cardiovascular Prevention and Rehabilitation Program Cardiac rehabilitation program designed specifically for people living with heart failure
 
Diabetes, Exercise and Healthy Lifestyle Program Program which combines exercise, education and healthy lifestyle behaviour changes to better support people living with diabetes or pre-diabetes. 
 
Health, Exercise, Active Living and Therapeutic Lifestyle (HEALTh)Program Education, exercise and peer support offered to assist women being treated for and recovering from breast cancer.
 
Heart Health for Life (Progression) Program offered to graduates of the cardiac, stroke, diabetes, heart failure or breast cancer programs * provide a variety of programs, services and resources to assist in the transition from formal rehabilitation to managing independently
 
Risk Factor Modification and Exercise After a Stroke Program (Progression) An exercise and prevention program designed for people who have had a stroke.
 
Women With Heart (Progression) Cardiac rehabilitation program designed specifically for individuals that identify as women
