Service Description: Supports for older adults, and adults with disabilities * helps people remain independent for as long as possible
 
Adult Day Program Adult day program * social, recreational, and therapeutic activities for adults and adults with disabilities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
 
Blood Pressure Clinic Trained volunteers measure seniors' blood pressure and provide educational materials and information on how to be more heart-healthy
 
Congregate Dining Group meals and social activities for older adults and adults with disabilities
 
Falls Prevention and Group Exercise Classes for Older Adults Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions * aims to improve strength and balance to prevent injury and falls
 
Home Help Assistance with daily chores, such as laundry, light housekeeping, errands, shopping, meal planning and meal preparation * home help personnel visit weekly, bi-weekly, monthly, or seasonally
 
Meals On Wheels Volunteers deliver hot or frozen meals to clients to help meet their nutritional requirements and provide security checks and social visits * hot meals delivered at lunch
 
Telephone Reassurance and Security Checks Regular visits and phone calls to check on the health and safety of older adults and adults with a disability people who live alone * screened volunteers listen, talk, and help isolated people stay in contact with the outside world
 
Transportation Services Non-emergency transportation to local medical and non-medical appointments and regular activities such as shopping, errands, and social outings * volunteer drivers * wheelchair accessible and ambulatory transportation is available in Dorchester, London, Thorndale and surrounding areas
 
Volunteer Visiting Regular volunteer visits to older adults and adults with disabilities * volunteer visitors are matched one to one with clients to provide companionship and support and may take place in-home or outside the home
