Service Description: |
Adult day program for people with acquired physical disabilities such as stroke, Parkinson's disease, Multiple Sclerosis * social, recreational, and therapeutic activities for adults with acquired physical disabilities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care * participation in this program will assist physical, social, emotional and cognitive development * transportation is not included in program fee Participants have the opportunity to engage in a variety of physical, social, and cognitive activities including:

Eligibility - Population(s) Served: | Older adults and adults 21 or older with acquired physical disabilities, who need physical, social, or emotional supports * Ontario Health atHome Care Coordinators assess the needs of applicants * must be living in the Mississauga/Halton area * program is not appropriate for people with a diagnosis of Alzheimer's disease or other dementias | ||
