Service Description:	Supports for the activities of daily living such as: living with stroke education and support group peer support program assessment services cognitive remediation heart healthy kitchen program horticultural therapy many exercise programs available aqua fitness outreach attendant services who visit in the home to assist with many activities of daily living access available for post stroke transitional care unit when transitioning from hospital to home, for short respite stays or for independent living assessments

Application:	Referral from North Bay Regional Health Centre (NBRHC)

Eligibility - Population(s) Served:	Adults and youth 16 years and older, and their families living with the effects of a stroke * valid health card required
