Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services
 
Adult Day Program for Frail Seniors Adult day program * social, recreational, and therapeutic activities for frail elderly adults in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
 
Adult Day Program for Seniors with Alzheimer's Disease and/or Dementia Offers social, recreational, and therapeutic activities for Alzheimer's disease and other related dementias in a group setting for part of the day * helps to support family caregivers who provide day-to-day care
 
Caregiver Support One-on-one and group support, as well as counseling for caregivers and families * family counselling * crisis intervention * support groups * training and education

Home at Last Program (HAL) Help older adults leaving hospitals to arrive home * offer clients additional support for up to 12 weeks after transitioning home from hospital
 
Home Help or Homemaking Assistance with daily chores such as laundry, light housekeeping, errands, essential grocery shopping, meal preparation and personal care * service is offered weekly, bi-weekly, monthly or seasonally
 
Meals on Wheels Hot meals delivered around 12 noon Mon-Sat or chilled meals delivered on Wed and Fri * special diet types available. Chinese and South Asian meals available.
 
Parkdale Assisted Living (PAL) A service that provides on-site support services to help older adults and people with disabilities with their daily activities
