Service Description:	Supports for people living with prediabetes or diabetes Offers education, prevention and self-management support to fit an individual's lifestyle Provides access to a registered nurse and registered dietitian Provides referrals to other health care professionals Programs and services include: Blood sugar monitoring Client support groups Community screening Foot care workshops/clinics Health fairs Presentations to the public and to health care providers School programs and visits Ontario Telemedicine Network (OTN) Provides access to specialized medical care using video conferencing and other tele-diagnostic equipment Allows specialists to remotely examine and prescribe treatments to patients in their home communities Reduces the need to travel to receive medical care
