Service Description:	Counselling and support for people with type 1 diabetes and type 2 diabetes (on insulin or insulin pump), gestational diabetes * help to patients and families to understand diabetes and how to self manage the disease - test blood sugars, take insulin, manage medications, make healthy food choices appointments conducted in person insulin pump program insulin adjustment classes (pattern management/30/70 class) gestational diabetes program transitional program for young adults moving from a pediatric program lipids class healthy kidney class weight management program satellite diabetes education centres (Bolton, Malton and Woodbridge) South Asian programs (Peel and Etobicoke) referrals to social worker and chiropodist (foot clinic) community diabetes seminars in-services to health care providers (hospital and community) Location: Fourth Level
