Case managers from Ontario Health atHome South West area available on site

Ambulatory Care Outpatient specialist clinics * diagnoses and treatments for various medical conditions

Complex Care Program Continuing medical management and a range of interdisciplinary, diagnostic, therapeutic and technological services * assessment, restoration, reactivation, palliative care and continuing care placement

ConnectCare Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response

Mental Health Care Building Inpatient and outpatient mental health services at the tertiary and long-term level

Rehabilitation Programs Inpatient and outpatient rehabilitation programs for people recovering from spinal cord injury, acquired brain injury, trauma, stroke, amputations, and neurological conditions

Specialized Geriatric Services Specialized care of older persons * evaluation and treating of functional, medical, and psycho-social aspects of illness and disability
