Service Description:	Provides education and services for the prevention, management and treatment of diabetes Provides case management and screening services Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes Advises on developing and implementing an approach to care and management of diabetes Assists individuals in finding appropriate services Provides referrals to other health care professionals Provides programs and services including: Annual poker walk for all ages with grand prizes Basic foot care clinics Breakfast and lunch programs Community kitchen Fitness and nutrition programs Glucose monitoring Pre-natal education School diabetes program Support groups
