| Service Description: | Provides ongoing and continuous support in the community for individuals living with severe and persistent mental illness. Offers support including: 24-hour crisis intervention, counselling, medication education, assistance with basic necessities of daily living (housing, food, finances), access to substance use services and criminal justice system. | ||
| Application: | Referrals accepted from self, hospitals, physicians, and community support services and family. Referrals from St. Joseph's Health Centre Department of Psychiatry are prioritized. Referrals are coordinated by The Access Point. Please call or complete the online application form. | ||
