Service Description:	Delivers medical transportation services and assistance in making medical appointment arrangements Provides transportation from/to Rainy River First Nations Reserve to any medical appointments (doctor, dentist, optometrist) in the towns of Emo and Fort Frances; exceptions for travel to Rainy River will be made on a case-by-case basis Provides assistance with completing prior approval applications for long distance medical appointments

Application:	Van bookings are to be arranged with Laura or Juanita Requires written proof of medical appointment Requires 24 hour advance booking; exceptions made for emergencies Advance cancellation recommended
Eligibility - Population(s) Served:	Members of Rainy River First Nations who require assistance with medical transportation due to illness, injury or age
