Eligibility - Population(s) Served: Anishinaabe individuals and family members accessing the Lake of the Woods District Hospital for medical reasons including:

  • Those who are accessing hospital services on an outpatient basis

  • Have recently been discharged or

  • Are family members of an inpatient who is seriously ill or receiving palliative care
