Service Description:	Operates a regional assessment and treatment centre for pre and post-operative care of patients with obesity requiring gastric bypass surgery Two types of services offered include: Medical Services Provides non-medical services, including nutrition and lifestyle counselling, for individuals who cannot or choose not to have surgery Surgical Services Performs gastric bypass surgery, however, will refer complex cases to a partner program in Southern Ontario

Application:	Individuals must have their primary care provider request a referral to the clinic through the Ontario Bariatric Registry A primary care provider can be a family doctor, a walk-in clinic doctor, or a nurse practitioner
Eligibility - Population(s) Served:	Individuals 18 years of age or older who are obese Individuals who are considering surgery are required to have a body mass index of 40 or higher
