Service Description:	Provides services and care to individuals who have physical disabilities, serious health issues, or those who are frail and/or elderly Assists individuals with the following: Access to cultural activities, Elders and Healers, traditional foods and ceremonies Advocacy with community agencies Assistance with housing issues Home visits/friendly visits Medical transportation and accompaniment services Preparing forms and applications Referrals to community services and Friendship Centre programs Security checks
