Service Description:	Provides service enhancement to keep people with mental illness and/or addiction issues out of the criminal justice system Offers short-term case management services Provides assessments and referrals Provides discharge planning Assists with housing and financial supports Provides access to resources in home communities Connects with clients prior to release or upon release to facilitate access to appropriate community services Liaises with partners involved in the criminal justice system
