Service Description:	Functions as a community-based intensive case management program Provides rehabilitation, treatment, and support services to individuals with severe and persistent mental illness Helps individuals to live successfully in the community Services are provided by a variety of healthcare professionals, including: Nursing Occupational Therapy Peer support specialist Psychiatry and family medicine Psychology Social Work Therapeutic Recreation
