Service Description:	Exercise and education for persons living with chronic pain Provides a 6-week outpatient program that focuses on skills, coping strategies, support, and education Provides an inter-professional team to help individuals set goals Note: A less intensive program is available for clients unable to attend the full 6-week program

Eligibility - Population(s) Served:	Individuals aged 16 years of age and older with chronic pain that interferes with daily activities Individuals who are ready to learn and practice self-management strategies
