Service Description: |
Provides health care services for older adults with, or at risk of, responsive behaviours/personal expressions associated with dementia, complex mental heath, substance use and/or other neurological conditions (such as early onset dementia) and support for family and professional care partners
Behaviours/personal expressions may be related to:
Regional Clinical Intake Team: Support referrals to BSO’s mobile community based teams and respond to general inquiries * supports coordinated access to geriatric consulting specialists and enhanced specialty care settings Clinical Behaviour Response Specialists: Provide direct behavioural assessment, treatment, transitional support and behavioural health care planning including potential causes of personal expressions/responsive behaviours to develop formalized care strategies with care partners Behavioural Support Facilitators: Provides support, education and care strategies to family caregivers of older adults with responsive behaviours and are linked with Alzheimer Society Chapters Transitional Activation Support Workers : Provide short-term support to older adults and their care partners to enhance successful and sustainable transitions from one environment to the next * employ activation and engagement to aid the person to feel comfortable in their new surroundings Psychogeriatric Resource Consultants: Provide education, indirect clinical consultation, network building and support for professionals working with older adults with responsive behaviours Dedicated BSO Staff in Long Term Care: Provide coaching/mentoring, behavioural health assessment and individualized non-pharmacological care strategies and are based in various long-term care homes throughout the North East |
