Service Description:

Community Support Services for older adults, adults with disabilities, and people recuperating from injury, illness and accident helping people to remain independent in their home of choice and community for as long as possible . Also provide information and referral to other community services

Bathing Program For those needing bathing assistance * therapeutic baths done by trained staff * can be scheduled weekly or as needed in hydrosonic tub at Ritz Villa/West Perth Village

Congregate Dining and Social Programs Group meals and social activities

Friendly Visiting Regular volunteer visits to older adults and people with disabilities * volunteers matched one to one with based on mutual interests

Meals on Wheels Hot meals delivered Mon, Wed, and Fri between 11:45am-12:30pm * special diet types available

Wellness In-home (one-on-one) and group programs including:

West Perth Transit Non-emergency transportation to local medical appointments and regular activities such as grocery shopping, errands, hair appointments, pharmacy, weddings, special family gatherings, the post office * volunteer drivers * wheelchair accessible vehicles * chartered booking also available
